Your Dietitian Nutritionist Credentialing Roadmap
Starting the credentialing process can feel overwhelming — especially with different licenses, payer rules, and state requirements. This page gives you a clear, simple path so you know exactly where to begin.
Whether you are a dietitian, nutritionist, CNS, or CNS candidate, follow the steps below to determine the best route for insurance credentialing and practice setup.
1. Identify Your Provider Type
Credentialing requirements and application pathways vary heavily depending on your specific board credentials, medical specialties, and state laws. Please choose the option that best fits you or your expanding group practice:
Nutrition & Dietetics Professionals: Registered Dietitian (RDN), State-Licensed Nutritionist (LN, LDN, CN, etc.), Certified Nutrition Specialist (CNS), or MS Nutrition Graduates.
Medical Providers: Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), Physician Assistant (PA), or Nurse Practitioner (NP, ARNP).
Behavioral Health Providers: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC/LMHC), Licensed Marriage and Family Therapist (LMFT), or Clinical Psychologist (PsyD/PhD).

2. Review State Licensure Rules
Your state determines:
what title you can use
whether you can provide MNT
which insurers allow non-RDN providers
whether CNS/CN/LN can credential independently

3. Choose Your Credentialing Path
Once your license and state allow it, select the route that matches your goals:
Solo Provider
Start credentialing under your own business or personal NPI.
Group Practice
Credential multiple clinicians under a single TIN and expand your team.
Transitioning from Berry, Fay, Vida, or Agency-Based Work
If you’re currently working under an agency’s contracts, start with our

4. Prepare Your Essentials
For All Provider Types (Universal Requirements)
National Provider Identifier (NPI): An active Type 1 (Individual) NPI, plus a Type 2 (Group) NPI if you are operating as a group practice.
CAQH Profile: A fully updated, completed, and unlocked CAQH profile.
Business Infrastructure: An established business entity (LLC, PLLC, or Sole Proprietorship) and a Federal Employer Identification Number (EIN).
EMR Setup: An active Electronic Medical Record system (Practice Better, Healthie, Kalix, Charm, or your preferred platform).
For Nutrition & Dietetics Professionals (RDN, LDN, CNS)
Dietitians - CDR Registration - Your active Commission on Dietetic Registration card.
CNS Nutritionists - Your CNS credential.
State Licensure/Certification: A state license (LDN, LN, CN) if practicing in a regulated state.
Malpractice Insurance (COI): Active professional liability insurance (typically minimum requirements $1M/$3M limits).
For Medical Providers (MD, DO, NP, PA)
State Medical License: An active, unrestricted license in the state(s) where you will practice.
DEA Registration & Controlled Substance License: Required for prescribing privileges with major networks.
Hospital Privileges / Admitting Agreements: If required by specific payers for your medical specialty.
For Behavioral Health Providers (LCSW, LPC, LMFT, PsyD)
Independent Clinical State License: Active, unrestricted state licensure allowing for independent practice (e.g., LCSW, LMFT, LPC, LMHC).
Malpractice Insurance (COI): Active professional liability insurance covering behavioral health services (typically $1M/$3M limits).
