Practice Setup Checklist for Nutrition Professionals

Starting your private practice as a CNS, CNS candidate, or dietitian doesn't have to feel overwhelming. Use this checklist to get your business set up legally, professionally, and ready to grow.

✔️ Step 1: Legal Business Formation

☐ Choose your business structure (LLC or Sole Proprietor)

☐ Register your business name with your state

☐ Apply for an EIN (Employer Identification Number) from the IRS

✔️ Step 2: Licensing & Credentials

☐ Obtain any required state licenses (e.g., some are certifications and others are LDNs in some states)

☐ Apply for Type 1 NPI (individual)

☐ Apply for Type 2 NPI (organization)

✔️ Step 3: Professional Coverage & Compliance

☐ Purchase liability insurance (e.g., ProLiability, Acord, CM&F, HPSO)

☐ Set up HIPAA-compliant communication tools (email, fax, EMR)

Note: You will know your system is HIPAA-compliant if they offer you a Business Associate Agreement to sign, otherwise, they are not HIPAA compliant!

✔️ Step 4: Financial Setup

☐ Open a business bank account (Remember, it has to be under your Business Name)

☐ Track expenses using bookkeeping software or Google Sheets

☐ Set up estimated quarterly taxes if applicable

✔️ Step 5: Technology & Operations

☐ Choose an EMR platform (e.g., Kalix, Practice Better)

☐ Create or customize your intake forms and charting templates

☐ Set up professional email (e.g., hello@yourpractice.com)
